How is it treated?

How is it Diagnosed?

Tooth fractures can vary in severity from minor enamel cracks to complete fractures involving the root and pulp. Diagnosis starts with patient history to understand the trauma mechanism or onset of symptoms like sharp pain during chewing, temperature sensitivity, or visible tooth damage.

Clinical examination is performed under good illumination and often involves drying the tooth surface for better visualization. Transillumination and dental explorers help identify crack lines or irregularities. Dentists may also employ a bite test (e.g., using a Tooth Slooth) to reproduce symptoms and localize the affected tooth.

Radiographic imaging plays a crucial role. Standard periapical and bitewing X-rays may detect vertical or horizontal fractures but are often limited for crack detection. Conebeam CT (CBCT) imaging is more sensitive for evaluating complex or deep fractures not visible on regular X-rays.

Pulp vitality testing helps determine the integrity of the tooth's nerve. A non-vital response may indicate pulp involvement or damage. Dye staining (e.g., methylene blue) may also highlight cracks.

Tooth fractures are classified based on their extent: craze lines, fractured cusp, cracked tooth, split tooth, or vertical root fracture. Accurate classification guides treatment, ranging from bonding and crowns to endodontic therapy or extraction in severe cases. Early detection improves the chances of tooth preservation, highlighting the importance of detailed examination and appropriate imaging.
