Symptoms

Causes

How is it Diagnosed?

Low back pain is one of the most common complaints in clinical practice and diagnosis begins with a comprehensive history and physical examination. Important considerations include onset, duration, character, radiation, aggravating/relieving factors, and associated symptoms like numbness, weakness, or bowel/bladder dysfunction (suggestive of nerve compression or cauda equina syndrome).

Red flags such as trauma, fever, cancer history, or weight loss necessitate urgent investigation. A physical exam assesses range of motion, spinal tenderness, neurological function, and straight-leg raising test (for sciatica). Imaging is not routinely indicated in acute nonspecific back pain but is considered if symptoms persist beyond 6 weeks or red flags are present. X-rays may reveal fractures or structural deformities. MRI is the preferred modality for evaluating disc herniation, spinal stenosis, infections, or tumors.

CT scans are useful for bony abnormalities. Electromyography (EMG) and nerve conduction studies may be indicated in radicular symptoms. Blood tests may be done to rule out infection or inflammation.
